Post Discharge Clinical Services
Post discharge services provide additional support and improve outcomes for trauma patients and their caregivers. Trauma Outreach Services provides access to resources, support, and programs that are specifically designed to assist patients and families as they navigate their recovery. Services include individual counseling for survivors and caregivers.
